Only orderable as a reflex from order BC (Culture-Blood w/ Sensitivity).
Identification of organisms present in positive blood culture bottles.
Where applicable, detection of antimicrobial resistance genes.

Performed reflexively on all positive blood culture bottles (first bottle per patient per unique Gram stain per 7 day interval).

Critical Value

Gram stain from positive blood culture bottles are considered critical values and notification is performed per policy.
Additional notification will not occur following BCID results.

Interpretation

Organism identification will be reported.
Antimicrobial resistance genes will be reported with interpretation.
For Staphylcoccus species, mecA and mecC genes will be reported as detected or not detected. Methicillin resistance may be predicted based on mecA/C gene detection.
For Enterococcus faecium/faecalis, vanA and vanB genes will be reported as detected or not detected. Vancomycin resistance may be predicted based on vanA/B gene detection.
For Enterobacterales species, CTX-M will be reported if detected, indicating the organism produces an ESBL enzyme.
For Enterobacterales, P. aeruginosa, and A. baumannii, carbapenemase genes (VIM, IMP, KPC, NDM, OXA23/48) will be reported if detected, indicating the organism produces a carbapenemase and is resistant to carbapenem antibiotics.

Rejection Criteria

BCID will not be performed on positive blood culture bottles with no organisms seen on Gram stain (false positive blood culture).

The Gram positive BCID panel tests for the following organisms: Bacillus cereus group, Bacillus subtilis group, Corynebacterium spp, Cutibacterium (Propionibacterium) acnes, Enterococcus spp, Enterococcus faecalis, Enterococcus faecium, Lactobacillus species, Listeria spp, Listeria monocytogenes, Micrococcus spp, Staphylococcus spp, Staphylococcus aureus, Staphylococcus epidermidis, Staphylococcus lugdunensis, Streptococcus spp, Streptococcus agalactiae (GBS), Streptococcus anginosus group, Streptococcus pneumoniae, Streptococcus pyogenes (GAS), pan-Gram negative, and pan-Candida species. The BCID-GP tests for the followiing resistance genes: mecA, mecC, vanA, vanB.
The Gram negative BCID panel tests for the following organisms: Acinetobacter baumannii, Bacteroides fragilis, Citrobacter spp, Cronobacter sakazakii, Enterobacter cloacae complex, Enterobacter spp, Escherichia coli, Fusobacterium necrophorum, Fusobacterium nucleatum, Haemophilus influenzae, Klebsiella oxytoca, Klebsiella pneumoniae group, Morganella morganii, Neisseria meningitidis, Proteus spp, Proteus mirabilis, Pseudomonas aeruginosa, Salmonella spp, Serratis spp, Sertia marcescens, Stenotrophomonas maltophiliam, pan-Gram positive, and pan-Candida species. The BCID-GN tests for the followiing resistance genes: CXT-M, IMP, KPC, NDM, OXA23/48, VIM.
